Coordinated Service Planning

The Coordinated Service Planning initiative is for children with multiple and complex needs. Coordinated Service Planning goes beyond the regular, interprofessional communication and collaboration among providers who are integrating practice and service delivery for children, youth and families. It is support intended to decrease family stress by providing a formal voice in the service-planning process and help to navigate and coordinate services.

The goal of Coordinated Service Planning is to support families and children/youth with multiple and complex special needs through a dedicated Service Planning Coordinator who will connect them to the various, cross-sectoral services they need, as early as possible, and monitor their needs and progress through a single Coordinated Service Plan.

The implementation of Coordinated Service Planning is well underway with lead agencies in place for all but two communities. The Purpose

The enactment models vary in each service delivery area, but the vision and goals are consistent. Families of children and youth with multiple and complex needs will be at the centre of coordinated service planning, and they will receive a single plan of care, that includes all services the child or family gains.

If you are feeling overwhelmed and need help to coordinate the services your child needs, or need additional supports, ask your ministry service providers about Coordinated Service Planning. You can also call the Coordinated Planning Service in your community. Contact information is available on the Ministry of Children, Community and Social Services at Service Delivery Areas and Coordinating Agencies.

In southwestern Ontario, the lead agency for London-Middlesex, Oxford-Elgin and Huron Perth has developed a great website for families. While some of the information is location specific, some information resources would be helpful to any family. A little about Alison

As the mother of two girls, one with cerebral palsy, I have been advocating in the school system for almost 25 years. I have a Bachelor’s and Master’s degree in Geography and never expected to become so involved with the school system and supports for students with disabilities.
